Hi Mark,

I was lucky enough to get one of these at an army surplus store where it
was mixed in with several boxes of manuals and training materials. I think
I only paid $4 or $5 for it and I have not seen one at that store since.
It is called an "EQUIPMENT RECORD FOLDER" and has U. S. ARMY embossed on
the front along with the us army seal and PROPERTY OF US GOVERNMENT. It is
just a rubberized binder with 4 clear plastic pockets in the middle and a
pocket on the inside of each cover. There is a clear plastic pocket on the
outside of the front cover that a 3 X 5 card fits into easily.

I looked and there is no FSN on it but it has the warning on the front in
letters hardly visible to the naked eye, "THIS RECORD IS A DEPARTMENT OF
THE ARMY CONTROLLED ITEM AND MUST BE SAFEGUARDED AGAINST LOSS AND DAMAGE.
IN THE EVENT OF LOSS SEE TM 38 750.

I hope this description is of some help to you and interest to others. I'd
be curious to know what that TM says about the folder. It is probably no
coincidence that the -10 (operators manual) to my truck fits in the pocket
of the front cover and the LO sheets fit in the clear vynal sleves. Good
