Joe,

         Correct me if I'm wrong but he sure sounded like he was describing
a surging rpm Diesel. Suggestion, check for entrapped AIR in the supply
fuel line. I guarantee it will cause rpm to rise with your foot off the
throttle. Other late thought, with the engine shut down verify the throttle
is returning all the way to the idle position (internal throttle return
spring may be broken FIX: Install external one) and that the idle screw
isn't wound all the way in if this truck is new to him.

         That's just off the top of my head Joe, maybe it will be of help.
