Mistakes happen, but this one doesn't qualify as a "mistake" by anyone's
yardstick. It's negligence. Chargeable negligence. It will likely come out
that everyone thought it was everyone else's responsibility but...no sale.
Ron's probably right about the likely CFIT finding. Anything for a
scapegoat.
