Hi folks,
Here's what I found last night, will mail copies today. Did not find the
one item I'd mentioned to you both, something on the order of "Equipment
Serviceability Criteria (or Schedule)", for time estimates of various
repairs to the M37 types (3/4-ton Dodge postwar). So, I'll continue to hunt
it down, and forward when I do.
Coming today:
- form DA 2408 Equipment Log Assembly (Records)-Dec '77
   (basically a set of directions for filling out forms 2404, 2408-1,
2408-12, 2408-13, 2408-14)
- form DA 2408-1 Equipment Daily Log-May '81
    (a basic service record...date,odom,oil,fuel,etc.)
- form DA 2408-5 Equipment Modification Record-Jan '64
    (tracks MWO activity)
- form DA 2408-9 Equipment Control Record-Oct '72
    (tracks acceptance,usage,loss,gain,or transfer between organizations,
this is a 3-part carbon form, replaces 2408-7 and 2408-8, both of Jan '64)
- form Da 2409 Equipment Maintenance Log-Apr '62
    (tracks maint., repair & cost, modifications)

All these reference TM 38-750 for instructions on usage of the forms, and
list proponent agency as DCSLOG (whatever that is??). First one includes
references to aircraft, so I'd guess they all apply to 'em.
Sending single-sided copies on 8 1/2" x 11" sheets, but the size of these
forms is:
6 7/8" (7" for the 2409) x 9 1/2",
3-hole punched for Equipment Log Book binders, on card stock (except carbon
form as noted). I noted whether my copies are front or back of form, if not
already stated on form.
