As far as starting from 2nd gear, I have always used the following method to
determine what gear to start with: If you need to overcome a spring, or
detent, to get into 1st, then it is a "low-low" gear, not to be used for
starting. I always start in 2nd, unless I have a heavy load, but even then,
by the time I get out of 1st, and into 2nd, I have lost a lot of momentum,
and it's almost like starting from 2nd.

I expect that there are as many opinions as there are people reading this,
